den, wird die Diagnose des „funktionel-len Sodbrennens“ gestellt (. Abb. 1; [11, 12, 13]).
Vor allem bei PPI-refraktären Symp-tomen nimmt die Funktionsdiagnostik einen zentralen Stellenwert ein. Die Wahl der Methode ist zum einen abhängig vom jeweiligen Zentrum und den Verfügbar-keiten vor Ort, zum anderen von der kli-nischen Fragestellung. Klinisch gilt es, den Nachweis bzw. Ausschluss einer GERD zu
führen oder eine mögliche Therapieopti-mierung bei bereits gesicherter GERD zu erreichen. Neben der herkömmlichen ka-theterbasierten 24-stündigen pH-Metrie nehmen die katheterfreie, kapselbasierte pH-Metrie mit verlängerter Analysedauer (BRAVO-Kapsel, Given Imaging) und die MII-pH einen festen Platz in der Reflux-diagnostik ein, die die Analyse nichtsau-rer Refluxepisoden auch unter PPI-Medi-kation ermöglicht.
Katheterbasierte und katheterfreie, kapselbasierte pH-Metrie
Die katheterbasierte pH-Metrie ist das methodisch älteste Verfahren und in der Praxis weit verbreitet. Sie stellt weiter-hin das Standardverfahren der Reflux-diagnostik dar. Gerade in der präoperati-ven Diagnostik vor einem chirurgischen Antirefluxeingriff ist die Durchführung einer pH-Metrie obligat [6]. Zur Durch-führung der pH-Metrie stehen 1-Punkt- und 2-Punktmessverfahren in Speiseröh-re und Magen zur Verfügung. Der Kathe-ter wird transnasal eingeführt, und nach Intubation des oberen Sphinkters die öso-phageale pH-Elektrode 5 cm oberhalb des gastroösophagealen Übergangs posi-tioniert. Zur genauen Lokalisierung des unteren Ösophagussphinkters sollte eine Manometrie erfolgen. Alternativ kann der gastroösophageale Übergang anhand des sog. pH-Sprungs ermittelt werden.
Als Maß für pathologischen gastroöso-phagealen Reflux wird der Bestimmung der prozentualen Säureexpositionszeit [“acid exposure time“ (AET)] der Vorzug gegeben, da sie anderen Refluxscores wie dem Johnson-DeMeester-Score [14] in der GERD-Diagnostik überlegen erscheint.

Die katheterbasierte pH-Metrie wird durch den Reiz des Nasopharynx vom Patienten häufig schlecht toleriert und ist mit einer deutlichen Einschränkung im Alltagsverhalten verbunden. Dies führt zu Ungenauigkeiten in der Interpretation des Tests. Die katheterfreie pH-Metrie (BRA-VO, Given Imaging) wird deutlich bes-ser toleriert; in einer Befragung zogen sie 96% der Patienten einer konventionellen pH-Metrie vor. Patienten beurteilten hin-sichtlich ihrer Zufriedenheit und gleich-zeitigen Einschränkung im Alltag die ka-theterfreie pH-Metrie im Vergleich zur konventionellen pH-Metrie als überlegen (. Abb. 2; [15]). Durch die verlängerte Aufzeichnungsdauer von bis zu 96 h kann mit der Kapsel-pH-Metrie die Sensitivität

cathteter was passed transnasally and the esophageal pHsensor was positioned 5 cm above the LES. The config-uration of the catheter provided measurements ofintraluminal impedance at 3, 5, 7, 9, 15, and 17 cm abovethe LES and pH at 5 cm above and 10 cm below the LESin the stomach.
Combined MII-pH data were analyzed to separatesymptoms into those associated with acid reflux, nonacidreflux or not associated with reflux. The symptom index(SI) was considered to be associated with reflux if asymptom was detected 5 minutes or less after the refluxepisode. The SI was quantified by the number ofsymptoms associated with reflux divided by the totalnumber of symptoms. Separate SI were determined foreach individual symptom if patients recorded differenttypes of symptoms and a positive SI (+SI) was declaredif Z50% (ie, at least half of symptoms associated withreflux).11
Prolonged monitoring was obtained while continu-ing acid suppressing therapy. Data from the impedancechannels and pH electrodes were sampled at a frequencyof 50Hz and stored on a portable data recorder (Sleuth;Sandhill Scientific). Tracings were downloaded into acomputer reviewed and the timing of meals, changes inbody position, and the time of symptoms recorded. Mealperiods were excluded from the analysis. The tracingsobtained during the study were interpreted using pre-viously described criteria12 and a dedicated softwareprogram (Bioview Analysis, Sandhill Scientific, Inc).
Abnormal distal esophageal acid exposure on therapywas defined as pH >1.6 in the upright position and >0.6in the recumbent position.13 Meals were excluded from theanalysis. The tracings were downloaded into a computerand changes during the study were also interpreted usingthe dedicated software program (Bioview Analysis, SandhillScientific, Inc). Each study was read twice, first by thegastroenterology fellow and then finally by the consultantgastroenterologist.
Statistical AnalysisData were analyzed using SPSS 12.0 Basic software
package. Normally distributed continuous parameterswere compared using Student t test or Mann-Whitneytest depending on the normality of their distribution.Differences in proportions were compared using w2 testand P values less than 0.05 were considered statisticallysignificant.
RESULTSData from 100 consecutive patients (77 female;
mean age, 54 y; range 17 to 85) who underwentambulatory combined MII-pH monitoring on PPItherapy were included in the analysis. The meanmonitoring duration was 20.0±SD hours. With regardsto the medical therapy, 48 patients were evaluated ontwice daily PPI while the other 52 were taking PPIb.i.d.+H2RA q.h.s. All patients were studied whiletaking in PPI and H2RA prescribed by their referringphysician. Forty-two patients (female 36; mean age, 53 y;
range 20 to 85) were studied on a PPI b.i.d. plus a bedtimeH2RA (Table 1) for at least 1 month. There were nodifferences in the age of patients taking PPI b.i.d.(54±16 y) and those taking PPI b.i.d.+H2RA(53±15 y). Also there was no difference in the propor-tion of female patients in the 2 groups (71% vs. 85%).
Gastric pH and NABNAB was noted in 64% of patients taking PPI b.i.d.
compared only with 17% in the group of patients on PPIb.i.d.+H2RA q.h.s. (w2 21.95, df=1, P<0.001) (Fig. 1).The percent mean (±SEM) time intragastric pH<4 forpatients on PPI b.i.d. was significantly higher (P<0.01)compared with patients on PPI b.i.d.+H2RA q.h.s. duringupright (29.1±3.0 vs. 18.3±2.9), recumbent (33.5±3.4vs. 12.5±3.1), and entire period (31.5±2.8 vs. 18.0±3.0)(Fig. 2).

on PPI b.i.d.+H2RA q.h.s. during upright (3.8±1.7 vs.2.2±1.0), recumbent (2.6±1.6 vs. 1.5±1.0), and entireperiod (3.3±1.6 vs. 1.9±1.0). There was no statisticalsignificance between the groups (Table 2).
SymptomsTwo patients recorded no symptoms on the study
day in each group. Of the 58 patients on PPI b.i.d., the SIwas positive for acid reflux in 6 (10%), nonacid reflux in15 (26%), and negative in 37 (64%). Of the 42 patients onPPI b.i.d. plus an H2RA none had a positive SI for acidreflux, 13 (31%) had a positive SI for nonacid reflux, and29 (69%) had a negative SI. There was no statisticalsignificance between the groups (Table 3).
DISCUSSIONOur findings support the concept that the addition
of a H2RA to twice daily PPI will enhance control ofgastric acid secretion. Only 17% of patients with thecombination therapy experienced NAB compared with64% in the PPI b.i.d. group. Combination therapy wassuperior in controlling gastric acid in the upright,recumbent, and the entire period. The study alsodemonstrates the high prevalence of NAB in patientstreated with PPI b.i.d. therapy. These results comparefavorably with the study by Xue et al14 in which theyfound that NAB occurred in 82% of their patients on PPIb.i.d. compared with 40% in those on combinationtherapy. Overall, the gastric pH was better controlledin our patients with the addition of an H2RA with asignificant difference in the pH control in both the uprightand recumbent position. The esophageal pH was found to
be lower in the combination group compared with thePPI only group; however, this did not reach statisticalsignificance and this may be due to the number of patientsin this study therefore causing a type II error.
Previous studies have suggested that the healing rateof esophagitis is directly related to the degree of gastricacid suppression, therefore suggesting the importance ofpH control and GERD healing.15 Kuo and Castell13
demonstrated that using twice a day omeprazole was asuperior regime in intragastric acid control comparedwith once a day therapy. A single daily dosing of PPI willeffectively relieve symptoms in most patients (60% to65%); however, some studies have shown that patientswith extraesophageal symptoms may require higher dosesof antisuppressant therapy to relieve symptoms effec-tively.16,17 In this study, 10% of the patients on a PPIb.i.d. had only a positive SI for acid reflux compared withnone in the combination therapy group. This was notstatistically significant, as the small number of patientswas likely causing a type II error. The SI for nonacidreflux and those symptoms not associated with refluxwere more similar. This finding suggests the addition of aH2RA maybe expected to improve symptom controlowing to the better gastric acid control compared withthat of PPI twice daily.
The mechanism of NAB is unclear, but previousreports have shown that PPI efficacy is reduced in theevening compared with the morning dose.18 It is importantwhen questioning a patient regarding the correct time fordosing as taking the PPI before dinner (rather than beforebedtime) has greater overnight pH control.15
A study by Fackler et al19 reported on 34 subjects(18 normal volunteers and 16 GERD patients) comparingPPI twice daily and the addition of H2RA at bedtime atboth day 7 and day 28. They suggested that patientstaking H2RAs developed tolerance after a short time andNAB was reduced only temporarily.19 In our study, allpatients were taking H2RAs for at least 1 month beforetesting, although we believe that a small percentage ofpatients may develop tolerance to these drugs. We haverecently reported that >70% of patients have sympto-matic improvement with long-term use of PPI/H2RAcombination.20
In conclusion, the addition of a bedtime H2RA toPPI b.i.d. reduces the percent time with intragastricpH<4 and also NAB. H2RA should be consideredas adjunct therapy in patients with persistent symptoms.The addition of an H2RA to PPI b.i.d. improved overallgastric acid control, decreased NAB, and decreasedthe likelihood of acid reflux as a cause of symptoms in
