1 Verwendungszweck
MutaPLATE® Aldolase B (TM) ist ein molekularbiologischer Test zur Untersuchung desA149P (G/C) – Polymorphismus in der hereditären Fruktoseintoleranz mittels Taq-Man -Technologie. Das Kit kann in „offenen“ real time PCR – Systemen mit entsprechendenWellenlängen-Messkanälen, wie z. B. RotorGene (Corbett Research), MX3005P(Stratagene), ECO (Amplifa), LightCycler (Roche), ABI 7300 (Applied Bioscience) oderSmart Cycler (Cepheid) verwendet werden.
2 Einleitung
Die hereditäre Fructoseintoleranz ist ein sehr seltener Enzymdefekt, der denFructoseabbau in der Leber betrifft. Die Aldolase B, die normalerweise das Fructose-1-Phosphat in Dihydroxyacetonphosphat und Glycerinaldehyd spaltet, fehlt. Stattdessen istnur die Aldolase A vorhanden, ein Enzym der Glykolyse, dessen Substrat Fuctose-1,6-Bisphosphat ist und das Fructose-1-Phosphat nur mit fünfzigfach geringererGeschwindigkeit spaltet. Das erste Enzym des Fructoseabbaus, die Fructokinase, istnicht betroffen, so dass die Fructose in die Leberzelle gelangt, phosphoryliert wird und dieZelle nicht mehr verlassen kann. Aufgrund des Enzymdefekts kann das Fructose-1-Phosphat aber nicht gespalten werden, sondern häuft sich an und hemmt Enzyme derGlykolyse, der Gluconeogenese und des Glykogenstoffwechsels. Die schon erwähnteAldolase A ist betroffen, ebenso die Fructose-1,6-Bisphosphatase der Gluconeogeneseund die Glycogenphosphorylase. Hypoglycämien können dadurch die Folge sein, da imHungerzustand das Glycogen dann nicht mehr oder nur noch vermindert abgebaut wirdund auch keine Glucose aus Aminosäuren oder Glycerin gebildet werden kann.

3 Testprinzip
MutaPLATE® Aldolase B (TM) real time PCR-Kit enthält spezifische Primer + weitereReagenzien für die Detektion des A149P (früher A149P) (G/C)-Polymorphismus in derhereditären Fruktoseintoleranz, wobei genomische DNA als Matrize dient. Dabei wirddieser variable Bereich durch die spezifischen Primer derart flankiert, dass mittels Taq-Man basierter real time PCR ein 300 bp großes Amplifikat entsteht. Die Standard PCR enthält zusätzlich zwei sequenzspezifische Oligonukleotide, welchejeweils mit Fluoreszenz-Farbstoffen markiert sind (TaqMan-Probes). Beide Sondenbinden an die amplifizierte Ziel-DNA, welche den zu untersuchenden SNP (singlenucleotide polymorphism) an Basenposition 448 enthält. Dadurch wird nach Anregung(geräteabhängig bei ca. 470 nm) ein Fluoreszenzsignal emittiert, welches mit deroptischen Einheit des verwendeten real time PCR-Geräts bei 510 – 530 nm bzw. 550-560nm detektiert wird. Die TaqMan-Sonde für das G-Allel (A149) ist mit FAM (510 nm, grün) markiert, dieTaqMan-Sonde für das C-Allel (P149) ist mit YAK (555 nm, gelb) markiert. Dieentsprechenden Detektionskanäle sind im verwendeten real time PCR-Gerät vor PCR-Testdurchführung unbedingt vorher einzustellen! Mit Hilfe dieser beiden spezifischmarkierten Sonden wird die Identifizierung des mit Fruktoseintoleranz einhergehenden G/C bzw. C/C Genotyps eindeutig möglich. Es können folgende Ergebnisse auftreten:

1. Homzygot G/G: Nur Anstieg des Signals der FAM-TaqMan-Probe (510 nm-Kanal),aber kein Signal im YY-Kanal (bei 555 nm). D.h. nur grüne Fluoreszenz.
2. Heterozygot G/C: Anstieg des Signals der FAM-TaqMan-Probe (510 nm-Kanal) undgleichzeitig Anstieg des Signals der YY-TaqMan-Probe (555 nm-Kanal). D.h. grüne +gelbe Fluoreszenz.
3. Homozygot C/C: kein Signal im FAM-Kanal (bei 510nm), sondern nur Anstieg desSignals der YY-TaqMan-Probe im 555 nm-Kanal. D.h. nur gelbe Fluoreszenz.

6 Lagerung und Haltbarkeit
Alle Reagenzien sollten bis zum unmittelbaren Gebrauch bei <-20 °C gelagertund dann schonend (bei 2-8 °C im Kühlschrank) aufgetaut werden.
Ein mehrfaches Auftauen/ Einfrieren der Reagenzien ist unbedingt zuvermeiden (bei Bedarf nach dem erstmaligem Auftauen geeignete Aliquotsherstellen und die Reagenzien dann sofort wieder einfrieren).
Während der PCR-Vorbereitung alle Arbeitsschritte zügig in einem Kryobehälter
(z.B. Light Cycler® Cooling Block) durchführen bzw. alle Reagenzien stetsgeeignet kühlen.
Detektionsmix stets vor Lichtkontakt schützen.
Alle Reagenzien können bis zum Ablauf des (auf der Kit-Packung angegebenen)Mindesthaltbarkeitsdatum verwendet werden.
7 Hinweise und Vorsichtsmaßnahmen
Nur zur in vitro Diagnostik verwenden.
Dieser Test muss durch speziell in Molekularbiologie-Techniken geschultesPersonal durchgeführt werden: Die Regeln der Good Laboratory Practice (GLP)müssen eingehalten werden.beim Umgang mit humanem Proben ist darauf zu achten, dass es sich umpotentiell infektiöses Material handelt. Die Komponenten vor Gebrauch vorsichtig durch Pipettieren (ca. 15x) mischen -nicht vortexen!Testkit nach Ablauf des Verfallsdatums nicht mehr verwenden.
Die Arbeiten zügig und gekühlt durchführen: auch alle Stocklösungen während desArbeitens kühlen.Die Kitkomponenten immer in aufrechter Position einfrieren.
8 Testdurchführung
Vor Beginn der Arbeiten sollte sichergestellt sein, dass die Arbeitsflächen undeingesetzten Geräte dekontaminiert sind. Die Kit-Bestandteile auftauen undDetektionsmixe (gelb und weiß) im Dunkeln aufbewahren. Die benötigte Anzahl PCR-Gefäße im Cooling Block vorstecken (die zusätzlichen Gefäße für die Kontrollen (rot,grün) berücksichtigen). Die DNA-Proben bereitstellen und gut mischen.
Enzym Mix (gebrauchsfertig) Dieser gebrauchsfertige Enzym Mix ist ca. 3 Monate bei –20°C haltbar; nach demEinfrieren kann der noch maximal 2 x bei 8°C aufgetaut werden, sofern er während desjeweiligen Arbeitens nicht länger als 1 h gekühlt gelagert wird.
Master Mix - Herstellung
Die unten stehende Tabelle zeigt die Zusammensetzung für 1 Reaktionsansatz (25 µl).
Zur Untersuchung mehrerer Proben werden die jeweiligen Volumina mit derProbenanzahl (incl. Kontrollen) multipliziert (zusätzlich zur Anzahl aller Ansätze solltezum Ausgleich der Pipettier-ungenauigkeit ein 10%- Überschuss mit einberechnetwerden) und in ein steriles Gefäß gegeben (Master Mix).

Die Lösungen sollten in der gleicher Reihenfolge, wie in der unten stehenden Tabelle,pipettiert werden.

Den fertigen Master Mix durch vorsichtiges Auf- und Abpipettieren mischen (ca. 15 –20x bis keine Schlieren mehr zu sehen sind) und in jede Kapillare 23 µl davon vorlegen.
Proben - Zugabe
Ab der 3. Kapillare je 2 µl Proben-DNA einfüllen. Die ersten beiden Kapillare solltenimmer für die Kontrollen (1. Negativ-Kontrolle, 2 µl und 2. Positiv-Kontrolle, 2 µl)vorgesehen sein.
Die befüllten Gefäße verschließen, kurz zentrifugieren und in das real time PCR - Geräteinsetzen.
Protokoll (MutaPLATE® Aldolase B TM) Alle PCR-Gefäße möglichst unverzüglich in das real time PCR - Gerät einsetzen und diePCR mit dem nachfolgend angegebenen PCR-Protokoll durchführen (entsprechend dasnotwendige PCR-Cycling-Programm bereits vorab im verwendeten real time PCR-Instrument hinterlegen).

9 Auswertung
Die Ergebnisse der Analyse des A149P Polymorphismus werden bei ca. 510 - 530 nm imFAM-/ Grün-Kanal, sowie bei ca. 550 - 560 nm im YAK-/ Gelb-Kanal angezeigt (diekorrespondierenden Kanäle sind im verwendeten real time PCR Instrument entsprechendvorzuwählen): Die Positivkontrolle enthält ein heterozygotes DNA-Template für die A149P Mutation,d. h. sie beinhaltet ein G-Allel und auch ein C-Allel. Die folgenden Abbildungen zeigen typische Beispiele für homozygote bzw.
heterozygote Proben bezüglich des A149P Polymorphismus im LC2.0 real time PCR-Instrument (Roche).

10 Troubleshooting
Kein Fluoreszenz-Peak bei der Positivkontrolle oder den Proben bei ca. 530 bzw. ca. 555nm:
Überprüfen Sie die Programmierung des real time PCR-Geräts. => Wiederholen Sie die Analyse mit korrigiertem Protokoll.
Der MutaPLATE® Aldolase B (TM) wurde mehr als 2x aufgetaut und wiedereingefroren oder war länger als 4 Tage Temperaturen von 2-8 °C ausgesetzt.
=> Beachten Sie die Lagerbedingungen. Wiederholen Sie die Analyse mit einem neuenMutaPLATE® Aldolase B (TM) - Kit.
Geringe Qualität der untersuchten DNA. =>Halten Sie sich exakt an die Herstellervorschrift zur DNA- Extraktion.
Schwacher Fluoreszenz-Peak bei ca. 530 bzw. ca. 555 nm:
Komponenten vor Gebrauch vorsichtig (nur durch Pipettieren) mischen: nicht vortexen!
Alle Stocklösungen während des Arbeitens kühlen, den Detektionsmix vor Lichtschützen.
Arbeiten Sie nur gekühlt, z.B. mit auf +4 – 8 °C vorgekühltem Cooling Block oder „aufEis“.

1 Intended Use
MutaPLATE ® Aldolase B (TAQ-Man) real time PCR kit is a molecular biological test foranalysis of the A149P polymorphism of the aldolase B Aldolase B bisphosphate ALDOBgene in open real time PCR systems (e. g. RotorGene, SmartCycler, Light Cycler, ABI,Stratagene, Amplifa).
2 Introduction
Aldolase B fructose bisphosphate catalyzes the reversible reaction of fructose-1-phosphate to glyceraldehyde and dihydroxyacetone-phosphate. Further conversion ofthese 3-carbon products allows the byproducts of fructose metabolism to enter into theglycolysis pathway. Homozygous loss of function of aldolase B leads to an inability of thebody to break down fructose into byproducts for human consumption. Due to thecontinuation of the first step in fructose breakdown, the conversion of fructose to fructose-1-phosphate, a phosphate sink is created in these patients, in which all the availablephosphate is consumed in the cell. This leads to the halt of gluconeogenesis andglycogenolysis. Symptoms usually begin at the time of weaning and include: bloating,abdominal pain, diarrhea, hypoglycemia, hyperuricemia, mental depression, jaundice, andcirrhosis that can lead to liver failure and death. Many patients display a food aversion tofructose-rich foods and practice strict avoidance of fructose-containing foods before theirdiagnosis. The current gold standard for testing of fructose intolerance is the hydrogenbreath test, in which the patient is administered fructose in the clinical setting, andhydrogen levels are measured before and at intervals after administration of the bolus offructose. An increase in hydrogen produced reflects the failure of the patient to absorbfructose, which is then metabolized by bacteria in the gastrointestinal system to give offhydrogen. The disadvantage of this test is that many patients respond to the fructoseadministration with signs of severe hypoglycemia including altered consciousness,diaphoresis, and seizures, sometimes resulting in death. Therefore, elucidation of thecauses of HFI and available genetic testing can avert the extreme clinical risks associatedwith standard of care diagnosis. There is currently no cure for HFI, with the only treatmentstrict diet modifications to avoid fructose-containing foods.
3 Principle of the test
The assay contains two sequence specific primers flanking the region of interest and twoTaqMan probes specific to the region containing the mutation. The two TaqMan probesare labeled at the 5' end with different fluorophores (reporter dyes) which are used for theallelic discrimination. On the 3' end the TaqMan probes are labeled with a non-fluorescentquencher. The proximity of the reporter dye to the quencher inhibits the fluorescence ofthe reporter molecule. During amplification the probes hybridize specifically to the DNAfragments. The 5' nuclease activity of the Taq polymerase cleaves the hybridized probesreleasing the reporter from the quencher generating a fluorescent signal. The TaqManprobe for the G-allele (wildtype) is marked with FAM (510 nm, green) and the TaqManprobe for the C-allele (mutation) is marked with YAK (555 nm, yellow).

The following three discriminations are possible:
1. Homozygous G/G:Increase of the fluorescent signal from the FAM labeled TaqMan probe, noincrease of the fluorescent signal from the YAK labeled TaqMan probe.
2. Heterozygous G/C: Increase of the fluorescent signal from the FAM labeled TaqMan probe andincrease of the fluorescent signal from the YAK labeled TaqMan probe.
3. Homozygous C/C: No increase of the fluorescent signal from the FAM labeled TaqMan probe,increase of the fluorescent signal from the YAK labeled TaqMan probe.

Each kit contains enough reagents to perform 32 respectively 96 tests. Each kit alsocontains a package insert.

7 Warnings and precautions
The regulations and principles for working in a biomolecular laboratory have to bestrictly followed.

8 Test procedure
Gently thaw all components on ice and gently mix them before use (do not vortex) andshortly spun down. Keep in mind to protect the detection mixes against exposure to light.During the PCR setup all reagents have to be cooled. For the amplification one PCR tubeis needed per sample plus additional tubes for the negative and positive controls. Thefollowing table shows the volume of each reagent per sample. The mastermix have to beprepared for the number of samples (incl. negative and positive control) (N) plus 10 % tocompensate inaccuracies.
Master mix preparation
Following table shows the composition for one reaction (final volume: 25 µl). For analysisof several samples in parallel, a master mix should be prepared in a sterile vialmultiplying each single volume by the number N of samples (incl. controls). Additionally,10% more volume should be calculated for reasons of inaccuracy. The reagents shouldbe pipetted in same order as indicated in the table:

The mastermix has to be carefully mixed through by pipetting up and down orinverting (do not vortex) and shortly spin down. Aliquot 23 µL into each PCR tube.For the negative control add 2 µL of the provided negative control (green).For the positive control add 2 µL of the provided positive control DNA (Red).Add 2 µL of each sample DNA to the corresponding PCR tube.

Results of the analysis for the A149P polymorphism are shown for at 510 - 530 nm /green and 550 - 560 nm / yellow (choose corresponding channel of your real time PCR
instrument). The positive control (red) contains template heterozygous for the A149Ppolymorphism (one allel carries the G, another allel carries the C).
Following figures shows typical examples for homozygous wildtypes as well as aheterozygous sample on the LightCycler 2.0. Use a appropriate color compensation file,if necessary e.g. LightCycler.

10 Troubleshooting
Problem Solution
No or low fluorescence peakwith positive control orsamples
Check PCR-program of the real time PCR instrument inuse and repeat with corrected protocol.
The Detection Mix was thawn / frozen more than twice orstored longer than four days at 2-8 °C. Repeat analysiswith a fresh aliquot or new Detection Mix.
Quality of DNA template is not sufficient. Use freshlyextracted DNA and measure the concentration/puritybefore use.
The Detection Mixes were not protected from light. Repeatanalysis with a fresh aliquot or new PCR reagents.
