harmloses Herzgeräusch
50 -70 % der Kinder haben ein harmloses Herzgeräusch
6-8 /1000 Kinder haben einen Herzfehler
2 /100 auskultierbare Herzgeräusch ist pathologisch

"It's Probably Nothing Serious, but...":
Reappraisal of the approach to the child with heart murmurs: is
echocardiography mandatory?
Geva T, Hegesh J, Frand M
Int J Cardiol 1988
The performance of cross-sectional echocardiography on every child with a
heart murmur poses a heavy load and economic burden on pediatric
cardiology services.
We prospectively studied 100 consecutive patients newly referred to our
clinic
50 patients "innocent murmurs".
Cross-sectional echocardiography confirmed the initial diagnosis in
48 patients (96%). Two patients had bicuspid aortic valve.
10 patients "suspected heart disease"
Cross-sectional echocardiography providing the diagnosis in
8 patients (80%)
40 patients "pathological murmurs"
. Cross-sectional echocardiography providing the diagnosis in
38 patients (95%)
Initial assessment of heart murmurs in children: role of complementary
diagnostic tests
Alvares S, Ferreira M, Ferreira H, Mota CR
Review Port Cardiol 1997
170 consecutive patients newly referred for evaluation of heart murmur
118 patients "innocent murmurs".
Cross-sectional echocardiography confirmed the initial diagnosis in
117patients.1 trivial PS
9 patients „possible pathologic"
Cross-sectional echocardiography: 3 possible pathologic. 6 innocent
murmur
43 patients "pathological murmurs"
. Cross-sectional echocardiography providing the diagnosis in
41 patients. 2 innocent murmur
Clinical examinations sensitivity 97.6.%, specificity 93.6%
positivie predictive value 95.3%,
negative predictive value 99.1%
Initial evaluation of heart murmurs: are laboratory tests necessary?
Smythe JF, Teixeira OH, Vlad P, Dem,ers PP, Feldman W
Pediatrics 1990
161 prospective patients
109 patients "innocent murmurs".
Cross-sectional echocardiography confirmed the initial diagnosis in
107patients.1 small VSD,1 small ASD
6 patients „possible pathologic"
Cross-sectional echocardiography: 3 innocent murmur, 2 pathological
murmur, 1 possible pathologic
46 patients "pathological murmurs"
. Cross-sectional echocardiography providing the diagnosis in
41 patients. 3 innocent murmur, 2 possible pathologic
Clinical examinations sensitivity 96%, specificity 95%
positivie predictive value 88%,
negative predictive value 98%
