Herr S.L., 76 Jahre, am 11.03.2008 überwiesen vom Hausarzt
Anamnese: unklarer Gewichtsverlust (10 kg in ca. 12 Tagen),Appetitlosigkeit, Durchschlafprobleme, Atemgeräusche,Bronchialinfekt vor 6 Wochen,Ex-Nikotinabusus 40 py
Körperl. Befund: exspirator. Giemen bds., Hyperpigmentierung der Handinnenflächen
CT: V.a. auf zentrales Bronchialcarcinom T3 N3 M1, ausgeprägte Lebermetastasierung, multiple retroperitoneale und pararenale Metastasen,Knochenmetastase re. Os ilii.
Bronchoskopie: exophytischer, kontaktvulnerabler Tumor mit subtotaler Stenosierung am Abgang des Mittellappenbronchus re.
Leberpunktion: kleinzelliges neuroendokrines Carcinom

Hyponatriämie
Häufigste Elektrolytstörung bei hospitalisierten Patienten:
<135 mmol/l bei 15-22% bei hospitalisierten, 7% bei ambulanten Patienten
<130 mmol/l bei 1-7% bei hospitalisierten Patienten

Syndrom der inadäquaten ADH-Sekretion (SIADH)
SIADH ist die häufigste Ursache einer Hyponatriämie.
Aber Hyponatriämie tritt oft auch in Verbindung mit ECF-Volumendepletionauf.
SIADH wurde erstmals bei Patienten mit Bronchialcarcinom beschrieben, bei denen ein physiologischer Stimulus für die ADH-Freisetzung fehlte (‚inadäquate‘ ADH-Sekretion).
Antidiuretisches Hormon (ADH) = Arginin-Vasopressin (AVP).
Zunächst Annahme, dass AVP-Sekretion bei SIADH unabhängig von der Plasma-Osmolalität erfolgt. Dies trifft bei etwa einem Drittel der SIADH-Patienten zu.

SIAD
(1) Beseitigung der auslösenden Ursache
(2) Beurteilung der Dringlichkeit: Schwere, Dauer, Symptome ja/nein
Akute (<48 h) symptomatische Hyponatriämie:
Anheben der Na+-Serumkonzentration um 1-2 mmol/l/h durch NaCl 3% Infusion, in den ersten 24 h um nicht mehr als 8-10 mmol/l, Furosemid (?).
Reduzierte Korrekturgeschwindigkeit, wenn die neuro-logischen Symptome der Hyponatriämie verschwinden.

SIAD
Hyponatriämie von langer oder unbekannter Dauer:
Wenn Na+-Korrektur >12 mmol/l/24 h, Risiko der osmotischen Demyelinisierung:
Zentrale pontine und extrapontine Myelinolyse:
Zunächst Lethargie und affektive Störungen, dann Mutismusoder Dysarthrie, spastische Quadriparese, Pseudobulbärlähmung.
Deshalb langsamere Korrektur: 0,5 –1,0 mmol/l/h, max. 8 mmol/l/24 h und 18 mmol/l/48 h,
Häufige (alle 2-3 h) Kontrollen, evtl. CT (Hirnödem)

SIAD
Asymptomatische Patienten mit chronischer Hypo-natriämie:
Langsame Korrektur.
Flüssigkeitsrestriktion, nach Abschätzung aufgrund der Urin- und Plasma-Elektrolyte.
Demeclocyclin 2 x 300-600 mg/d. Hemmt AVP-Wirkung, →nephrogener Diabetes insipidus. Effekt variabel, nephrotoxisch. Li+ nicht mehr empfohlen.
Conivaptan (Vaprisol®), FDA-Zulassung für euvolämische (2005) und hypervolämische (2007) Hyponatriämie.

Conivaptan, ein nichtselektiver Vasopressin-Antagonist,
eine Option für mäßige bis schwere Hyponatriämie mit Symptomen, aber ohne Krampfanfälle, Delir oder Koma,
bei hospitalisierten Patienten.
UAW: Lokale Reaktion an Infusionsstelle (50%).
Randomisierte placebokontrollierte Doppelblindstudie: Anstieg der Na+-Serumkonzentration um 6 mmol/l.
Oral anwendbare selektive V2-Antagonisten werden entwickelt.
